Welcome to our forms section at the University Counseling Center. We are moving towards
a self-service system, where clients are able to begin processing/filling out their
required UCC packets before they physically come in to our office. Please note that
this is an optional process as you can still walk-in to our office and fill out the
necessary documents. The objective of beginning the process ahead of time is to expedite
the process and eliminate wait-times for you, as well as other clients coming in for
their respective sessions.
