- Registration Fee Assessment Form ((every semester you are applying for benefits; located at My TSU Web)Note: after classes begin, you need to submit Current Term Statement)
- Degree Works (every semester you are applying for benefits; located at My TSU Web)
- Copy of Other College Transcripts / Military Transcripts (JST)
- Residency Questionnaire (needs to be completed annually if you are an Out of State Student)
